We ask that each child attending Sunday School be registered by a parent or guardian. To register your child, please complete and submit this form, which will be sent to the Parish Office.  We also ask that you download and complete an emergency contact form, which you can mail or bring to the parish office or give to one of your child's teachers.

These forms are used to develop class rosters, contact lists, and attendance records.
